The Kent State Institutional Animal Care and Use Committee (IACUC) forms are designed to allow research investigators to address animal welfare considerations as well as other information required by the PHS Policy and USDA Regulations. The applications are used by the IACUC to evaluate the research with respect to the care and treatment of animals. It is also used by various regulatory agencies, e.g., the USDA, to evaluate the compliance of the institution with the various policies and regulations. Therefore, investigators are required to complete all sections of the IACUC forms according to the instructions on the them, and, in so doing, provide all the information necessary to all these purposes. Failure to include required information can result in delays for gaining IACUC approval.

Please Note: If you will be submitting a request that requires the use of hazardous materials, please include copies of all relevant MSDS along with your submission.


Request to Use Animals

Complete this form to be sure that the regulatory requirements are met when live vertebrate animals are to be used in a project at Kent State University. IACUC must review all research, teaching, and testing activities involving the vertebrate animals.

Participant Qualifications

If you are seeking approval for a protocol that will have more than four persons working on the project, you must complete this form, attach it to the protocol request, and submit it to the IACUC coordinator before IACUC approval of the protocol can be granted. All study personnel must provide documentation of IACUC training and enrollment in the occupational health program.

Addition of Personnel

Complete this form for each person that you want to add to an approved protocol that involves the use of animals. All new study personnel must provide documentation of IACUC training and enrollment in the occupational health program.

Modification of Existing Protocol

You must complete this form if you want to propose procedural changes or additions that involve living animals to an approved "Request to Use Animals" protocol.

Annual Review

You must complete this form annually if you are seeking renewal or completion/termination of a "Request to Use Animals" protocol. If your project continues longer than three years, you must complete a new "Request to Use Animals" form and submit it to IACUC before the three-year anniversary of the original approval. Requests must be submitted to IACUC at least 30 days prior to protocol expiration date.

Project Complete/Terminated

Complete this form if you are completing or terminating a protocol.
