The IRC needs to update Department Charge Authorization Forms to verify account numbers and authorized users. Please complete the below form (one per account number). This information is required prior to charging your department for instructional materials. The IRC can only charge to departmental accounts, not grants. The original departmental contact is able to add or delete names by stopping by the IRC or through email. We will no add people by phone, it has to be in person or in writing.
Only those persons listed in the below form will be allowed to make charges.
