This policy applies to all students enrolled in BSN, MSN, DNP, and PhD programs and postmaster's certificates. For admission to, and progression in the College of Nursing, students must provide documentation of the following items to Certified Background, Inc., a third party vendor who is approved to track and maintain student health and other legal documents. Students are responsible for all costs incurred in meeting the requirements for clinical compliance and for establishing and maintaining their Certified Background profile.

Components of Certified Background Profile

  1. Current American Heart Association Basic Life Support for Healthcare Provider certification in cardiopulmonary resuscitation (CPR). PhD students must have CPR certification within the first semester of enrollment.
  2. Background Check: Students are required to submit Federal and State criminal background checks through Certified Background and incur the related costs.   3. Healthcare Insurance: Students must provide proof of health insurance. Students can submit either a copy of an insurance card, if the student's name appears on the card, or a copy of the health insurance statement coverage, which indicates that the student is covered.
  4. Immunizations/Immune Status Testing: Evidence of the following immunizations/tests must be provided on the Immunization and Physical Compliance Form available from the Office of Student Services. The form must be completed, dated and signed by a licensed health care provider.
    1. Mantoux tuberculin (Tb) skin test: Upon admission students must obtain the TB test using the two-step method or QuantiFERON blood test. The two-step method means that student will receive a first Tb skin test and, one week later, will receive the second Tb skin test. Results of EACH step must be submitted.
      1. Results of a one-step Mantoux Tb skin test or QuantiFERON blood test must be submitted each year thereafter while the student remains in the nursing program.
      2. If the student has a positive reaction to Tb, he/she is required to have a chest X-ray, or indicate date of last chest x-ray if it was completed within the past one year. Each year thereafter the student will be required to complete an Annual Mantoux/TB Screening Form, which can be obtained from the Office of Student Services.
    2. Rubeola (Measles), Mumps and Rubella (MMR): A positive titer is required for each disease. A negative titer will require the administration of an MMR booster.
    3. Adult Diphtheria Tetanus: A Tdap is required, regardless of interval since last Adult Tetanus. Adult Tetanus is only accepted with proof of a Tdap. Both vaccines are valid for 10 years.
    4. Hepatitis B vaccination: This vaccination is highly recommended. Dates of all three injections are required. A titer-indicating immunity is also acceptable. Hep B booster acceptable with negative titer. If the student chooses not to receive the Hepatitis B vaccination, the waiver section must be signed and dated. Since Hepatitis  B is a series of three injections over the course of several months, it may not be completed before the deadline. Students should start the series immediately and submit an updated Immunization and Physical form once future injections are completed.
    5. Varicella: A positive titer is required. A negative titer requires proof of two vaccines administered in the lifetime.
    6. Flu Shot: Annual Influenza vaccine is required regardless of clinical rotation status. A licensed clinician-signed Influenza waiver is acceptable for allergies at some clinical agencies.
    7. Drug Toxicology: Some clinical agencies require 10-panel drug screen. Drug screens are valid for 1 year only. Students will be contacted prior to beginning clinical at an agency that requires a drug screen.
    8. Physical examination is required within one year prior to admission to a clinical site. For students admitted into the BSN, MSN and DNP programs, and postmaster's certificates, the provider conducting the physical must indicate that the student can perform activities in the nursing student's clinical and practicum courses, or can do so with reasonable accommodations, and is free of all communicable diseases. Students should request accommodations from Student Accessibility Services. Failure to obtain the physical examination or the provider assurance may prevent the university from placing the student in a clinical setting.

Students who have allergic reactions or other physiological conditions that preclude them from receiving vaccinations are required to contact the Office of Student Services upon admission to their academic program. Such conditions may prevent students from completing clinical/practical coursework at some partner agencies.

RN Licensure: Students enrolled in the RN-BSN, MSN, and DNP programs or post-master's certificates must have an active, unrestricted RN license in either: the State of Ohio for students taking on-campus courses or who will be completing practica in Ohio or in the state of residence and practice for applicants residing and completing practica outside of the State of Ohio. Students enrolled in the PhD program must have an active unrestricted Ohio RN license, or for intemational students, proof of legal ability to practice as a RN in country of origin. For students in the RN-BSN, MSN, and DNP programs, and post-master's certificates, proof of the appropriate licensure must be obtained within the first semester of the student's program, or prior to beginning practicum hours, whichever comes first. For students in the PhD program, proof of licensure must be shown at the time of admission into the program.
